How does the US electoral polling work?

Whenever I see a map of the current US polling it shows states which are voting Republican, Democrat and undecided. I have two questions, firstly does a third party never make up even a single state? And secondly, as I'm uming this is only a projection do these states ever change on the actual day of election? THANKS!
